Scheduled Appointments
You are very important to us and we strive to provide you with efficient and effective care. These guidelines have been created to clarify our appointment policies, which makes it easier for you to schedule appointments.
Office Hours
Monday - Tuesday - Wednesday: 7:00 a.m. - 5:00 p.m.
Thursday: 7:00 a.m. - 11:30 a.m. *Note we are open on selected Thursday afternoon's from 1:00 p.m. - 5:00 p.m.
Closed for Lunch: 12:00 p.m. - 2:00 p.m.
Friday, Saturday and Sunday: Closed
House Calls System
Our office uses a computer automated "House Calls System", which calls the night before to confirm the patient’s scheduled appointment. If you’re not able to attend the scheduled appointment the next day PLEASE call our office the evening before and leave a message cancelling the scheduled appointment. This will avoid any missed appointment charges. This is a "COMPLIMENTARY SERVICE" it is your responsibility to remember your appointment dated and time!
Long Appointments
Large procedures will have to be scheduled during work or school hours. There are many situations that require longer appointment times, including repair of loose and broken appliances which will need to be scheduled only at available times.
The Importance of Timeliness
Everyone’s time is important and we appreciate your being on time. It will help us stay on schedule and not have to reschedule your appointment.
After Hours
If you need to get in touch with our office after business hours please leave a message and we will return your call on the next business day. There will be an emergency number available on weekends for TRUE medical orthodontic emergencies.
Your Family Dentist
It is extremely important that you continue to see your family dentist every six months or as often as your dentist advises while you’re in an orthodontic appliance. This helps reduce potential problems like cavities and calcification build-up.
Additional Charges
Returned Checks -
Any returned check(s) will be assessed a $35.00 non-sufficient fund charge.
Late Charges -
A $25.00 late fee will be assessed on all accounts for payments received after the LAST day of the month.
Missed Appointment Charges -
If you need to change or cancel an appointment, a 24-hour notice is required. A $25.00 charge will be assessed on patient's account for all missed appointment.
Lost/Broken Retainers - Standard Hawley retainer $195.00 per retainer. Hawley retainer with tooth/teeth $225.00 per retainer. Essex retainer $90.00 per retainer.
Replacement Retainers due to cosmetic work - (see above lost/broken retainer fees)
Retainer Repair - $45.00
Retainer Insurance - Standard Hawley retainer insurance $65.00 per retainer. Hawley retainer with tooth/teeth $85.00 per retainer. Essex retainer insurance $40.00 per retainer.
Lost/Broken Positioner/Ortho Corrector - $300.00
Lost/Broken Headgear - $100.00
Lost/Broken Lingual Arch Wire or Bonded Retainer - $75.00
Loose/Broken Brackets - After the 3rd breakage $25.00 per bracket
Lost/Broken R/W/B appliance - Cost of outside lab
Clear Brackets - $450.00 per arch
Removing & Replacing appliance for special events - $70.00 per bracket
Office Visit - $50.00 per visit after treatment completed
Outside Services - such as diagnostic x-rays, extractions, surgeries, etc.
